Update: I was able to generate a good print profile from a print using the adobe print utility.  There is another option to print profile targets without color management in CS5 called "null transformation" method.  This method was developed by andrew chan of adobe labs.  Either way works.

I still think Adobe are using a tatty sticking plaster to solve it - all that money for a program that can't, in it's "native" form, do a very basic photographic task of printing a target. Lightroom can't either I'm told by my custom profile supplier.
