I've used this in the past with the same version of photoshop without issue, (CS6). As you can see below, when I export via photoshop "file/automate/Qimage Ultimate" the image is corrupted.  Opening the same image via QI and it's fine.

Open via browsing to image in QI  (raw0002-1.tiff)

I would appreciate having the bad qex file. I would see that Mike gets it ASAP.

Maybe put both the raw002.tif and the qex on Google drive and send us a link.  That way we could reproduce the problem from the beginning.  It looks like it might be some issue with the color mode on that particular TIFF or perhaps a layer type that isn't supported by the plugin.
