I turn out 2 or 3 a week - my method doesn't use cutouts:
Set the print size to 45x35mm - I also use a 5mm white border outside and
corner marks to make trimming easy.

In the page editor with 'Rule of Thirds' on the distance between the top of
the frame and the second 'Thirds' line is 30mm - exactly right for the
crown-to-chin measurement, which has to be between 28 and 35 mm - its easy
to do this -see

then all you do is drag the image down

The finished result:

I've saved this as a template and it makes passports really easy. We do
several a week, and without this it would not be worthwhile.
